Lead innovative community nursing care with the Virtual and Integrated Community Care Project.

Your new role:

Be a clinical leader in delivering innovative community nursing care through the Virtual and Integrated Community Care (VICC) Project, enhancing patient care across hospital and community settings.

  • Provide advanced clinical expertise, consultancy, and leadership within Acute and Community and Oral Health services.
  • Lead and coordinate the implementation of a consistent intake process and referral pathways across community health services.
  • Deliver direct and indirect person-centred nursing care ensuring safe, quality, and evidence-based practice.
  • Collaborate with multidisciplinary teams and stakeholders to optimise patient care pathways aligned with the SAFEST Patient Journey Home Framework.

About you:

You are an experienced and compassionate clinical nurse consultant, skilled in community nursing, leadership, and driving service improvements.

  • Registered Nurse with advanced clinical knowledge and demonstrated expertise in community health nursing.
  • Proven leadership abilities in managing clinical teams, coordinating care, and driving quality improvements.
  • Strong communication and interpersonal skills with the ability to engage effectively with multidisciplinary teams and stakeholders.
  • Commitment to continuous professional development, evidence-based practice, and person-centred care.
