Associate Clinical Nurse Consultant

Your new role:

Be a clinical leader in delivering innovative community nursing care through the Virtual and Integrated Community Care (VICC) Project, enhancing patient care across hospital and community settings.

Provide advanced clinical expertise, consultancy, and leadership within Acute and Community and Oral Health services.

Lead and coordinate the implementation of a consistent intake process and referral pathways across community health services.

Deliver direct and indirect person-centred nursing care ensuring safe, quality, and evidence-based practice.

Collaborate with multidisciplinary teams and stakeholders to optimise patient care pathways aligned with the SAFEST Patient Journey Home Framework.

About you:

You are an experienced and compassionate clinical nurse consultant, skilled in community nursing, leadership, and driving service improvements.

Registered Nurse with advanced clinical knowledge and demonstrated expertise in community health nursing.

Proven leadership abilities in managing clinical teams, coordinating care, and driving quality improvements.

Strong communication and interpersonal skills with the ability to engage effectively with multidisciplinary teams and stakeholders.

Commitment to continuous professional development, evidence-based practice, and person-centred care.
