**The Opportunity**

This leading not for profit organisation is seeking a Case Worker to join their Foster Care Team. You will be working alongside the Carer Support team to ensure the safety and wellbeing of young people in Foster Care by providing person-centred case management. This position will be based in the Inner West and will require travel to the Northern Beaches as required.

The case manager will provide and/or facilitate the provision of referral, assessment, general support, community placement, supported accommodation, outreach and aftercare services.

You will have the primary accountability of the client (young persons) case management - including coordinating specialist supports, allied health appointments and anything else relevant to their client.

**The Duties**:

- Engage in effective case planning, case plan development and case plan review
- Provide person-centred case management to clients
- Monitor consistency of person-centred plans in place according to individual clients’ support needs (including general support plans, behaviour support plans, health support plans)
- Meet regularly with Carers and the Carer Support team so that young people's wellbeing is actively safe guarded
- Work closely with the Carer Support team to monitor the progress in the current placement as well as planning for future placements
- Ensure foster and kinship carers are supported to provide high quality care and positive outcomes for the children and young people in their care
- Establish collaborative relationships with a wide range of specialist and mainstream services such as government and non government agencies, to improve outcomes for young people and their families
- Participate in a rotating on call roster

***Requirements**:

- Minimum Diploma qualification in Community Services or related field
- Experience of working with children and young people, and people at risk of homelessness highly desirable
- Minimum 1 year of experience in Case Management in OOHC
- Must have your own drivers licence, use of own vehicle and comprehensive motor vehicle insurance
- Knowledge, skills and experience working with young people and their families in a respectful and caring manner

A current working with children check (WWCC), police check or willingness to obtain and drivers licence are required to be successful in this role
