- Salvation Army Youth Family Services is committed to working in partnership with young people aged 16-25 to address the issues contributing to their unique experience of crisis & homelessness. The program is operated under the framework of a Psychologically Informed Environment, with a particular focus on assisting young people to:

- Develop life and living skills to prepare for independence
- Address issues in relation to health & wellbeing
- Promote opportunities to overcome exclusion and isolation
- Explore education pathways and participation
- Work towards long term sustainable housing
- About the role
- We are seeking an enthusiastic and experienced Case Manager to deliver high-quality case management support services to young men, women, and families, including those with accompanying children who are at risk of or experiencing homelessness. This is a maximum term (12 month), full-time position, 38 hours per week based in St Kilda at the Upton Road Youth Refuge.
- Salary and conditions are in accordance with the Social, Community, Home Care and Disability Services Award Level 5.
- You will have
- Relevant Bachelor or higher qualification in welfare or social work is required
- Demonstrated experience and highly developed skills in working with people who have multiple and complex needs
- Experience in working with young people who have complex trauma and attachment injuries
- Be proactive and self-motivated
- Be able to convey information effectively - verbally and in writing
- A current Victorian Drivers License is essential
- A current and valid Working with Children's check
- Willingness to undergo a National Police check

- Build effective relationships with clients and encourage self-determination and participation
- Provide ongoing assessment of the client's issues, needs and circumstances from initial intake to exit
- Develop individual case plans in collaboration with clients to meet a range of interventions needs
- Assist clients to access support services and establish community supports
- Regularly monitor and review client's individual case plans meeting needs or goals
- Develop implement and review therapeutic case plans with the Client Services manager
- Maintain accurate and up to date client records using TSA information systems
- Engage and work with young people with complex support needs
