**Position Overview**:
The Care Coordination Lead for CARED will play a pivotal role in ensuring the seamless delivery of healthcare services between clients and service providers. This role oversees all aspects of client operations, including client relationship management, service coordination and recommendation, and clinical quality assurance. The Care Coordination Lead will be the key point of contact for CARED and will ensure a high quality of customer service for both clients and service providers.

**Key Responsibilities**
- Connect clients with their appropriate service providers and recommend additional services
- Develop and deliver the onboarding process for service providers
- Ensure services are tailored to meet the needs and preferences of our clients
- Support recruitment efforts of suitable providers
- Contribute to the development and implementation of strategic initiatives to drive business growth and enhance client satisfaction
Lead the testing of the CARED platform across iOS, Android and Web.**About You**
- Allied Health Professional with min 3 years experience ideal
- Knowledge of multidisciplinary teams and appropriate intervention
- Proven experience in operations, preferably in the healthcare industry
- Proven track record in effectively documenting participant notes
- Thorough knowledge within the NDIS is highly desirable
