DSA has an exciting and unique opportunity for a Co-ordinator to work within our Individualised Living Options (ILO) program.

We are looking for people who live in the Inner West or South East Sydney, who want to work in the area they live in, and support people to connect with their communities.

**About your Role**:

- Provide support and assist participants and their families to develop a plan that best suits their individual needs
- Connect with community groups and people we support and their families to work towards meeting individual goals and outcomes
- Monitor and assess the funding and services which are available to the participant to get the best out of their plan
- Regularly review and refine plans and help the participant understand their rights and build their confidence and ability to make informed decisions
- Liaise and work collaboratively with key stakeholders in developing effective resources relevant to the achievement of a Participants NDIS goals

**About You**:

- You will have a passion to support people living with a disability to live in their own home in an inclusive way which meets the individual needs of each person
- Be an practice leader with proven experience in managing a diverse team and knowledge of the community sector
- Strategic in your thinking about how to improve service and contribute to diversional and organisational changes that positively impact your services
- You appreciate change and work well in leading a team through an ever changing environment
- Experience within the disability field or Tertiary qualified is highly desirable
