Axis Supports, an NDIS Registered Provider, seeks an experienced Support Coordinator or Specialist Support Coordinator to join our boutique but growing team. NSW-based applicants are encouraged to apply, as this role includes opportunities for monthly travel to Canberra with all expenses paid (accommodation, travel).

**Key Responsibilities**:

- Coordinate and manage support for NDIS participants, ensuring services align with their NDIS plans and goals.
- Work with multidisciplinary teams to build client capacity.
- Triage new referrals, and where necessary, recommend alternative service providers based on local knowledge.
- Mentor team members and contribute to business development efforts.
- Engage with your network to help grow our client portfolio.

**Qualifications & Experience**:

- 2+ years of experience in a Support Coordination role within the NDIS.
- Experience supporting individuals with complex disabilities, mental health conditions, or psychosocial disabilities.
- Strong communication, time management, and organizational skills.
- Proficiency with computer systems and CRM tools like Proda.
