**SERVICES COORDINATOR- SUPPORT AT HOME**

Gratitude Home Care is seeking a stand-out Service Coordinator, Home Care Package (HCP), to join the client services team in our Home Care Services department and responsible for delivering an exceptional client experience through a combination of service coordination, relationship management, and proactive customer care. The role includes handling client incidents, feedback, scheduling and contributing to improvements in client experience and business processes.

**Key Accountability**:

- Coordinating all aspects of Home Care Packages and Support at Home clients within All states
- Optimising care within clients identified care needs (liaising with Clinical Care Coordinator, Allied Health and key stakeholders)
- Responsible for ongoing care planning, assessment and reviews
- Responsible scheduling based on service needs
- Answer new potential client enquiries regarding available service provision and pricing
- Strong ability to manage client relationships and expectations with outstanding customer service
- Demonstrated professionalism in communication with clients, families, colleagues, suppliers, and other stakeholders
- Ability to prioritise, problem-solve, and manage urgent or high-volume tasks efficiently
- Commitment to continuous improvement and contribution to a culture of service excellence
- Understanding of the aged care system appropriate to the role, including Home Care Packages, Support at Home, Aged Care Quality Standards, and the Aged Care Act
- Ability to prioritise tasks and follow through to completion.
- Ability to work both independently and collaboratively to achieve team and organisational objectives.
- High attention to detail, problem-solving skills, and ability to prioritise tasks
- Ability to document and communicate client needs clearly to stakeholders
- Flexibility and adaptability in a changing environment, embracing continuous improvement

**Desirable**:

- Experience in community-based aged care or health-related fields
- Background in customer feedback and complaint resolution
- Experience in fast-paced scheduling environment

Pay: $56,515.49 - $65,645.86 per year
