**About the role**:
Due to business growth, you will manage a small portfolio of community-based elderly customers (with home-care packages) and focus on signing up new referrals to KinCare. The key responsibilities include a sales focus, building relationships with customers and their families, developing tailored care plans, managing budgets, and coordinating care services to maintain our customer's independence. You will be inspired and motivated to make a positive difference to the families and achieving ultimate customer satisfaction.
Your strong account management and networking skills will enable you to build rewarding external relationships within the relevant communities. Your week will be spent mostly out on the road (around areas such as St Morris, Glenelg, Glenside) visiting customers in their homes, while the rest of your time will be spent liaising closely with our internal teams to ensure the smooth delivery of care services.
Our office is in Wayville but you won't need to be in too often. This is a flexible and autonomous role where you’ll have the ability to manage your own schedule, including working from home where needed. It's a purely case management role with no scheduling or management of support workers (we have other teams to do that).
**About you**:
- Previous case management experience especially Home-Care Packages.
- Advanced knowledge of the Community/Aged Care system and funding model.
- A current valid driver's license and reliable vehicle.
- Strong coordination and organisation skills.
- Exceptional customer service, negotiation and influencing skills.
- Excellent ability to connect with multiple stakeholders and think commercially.
- Experience using a customer database or online CRM tool.
- A clinical background (RN, allied health, social work, dietician, mental health worker, etc) would be great but not essential.
