**Description**:
Key Responsibilities
Lead and supervise Care and Support Workers, providing guidance, training, and performance feedback.
Ensure quality care delivery that meets individual client needs, promotes independence, and upholds Coolibah Care’s values.
Assist in onboarding new clients, helping them transition smoothly to permanent care arrangements.
Support and collaborate with Care Partners to review and adjust Support Plans.
Maintain accurate documentation and uphold privacy and confidentiality standards.
Advocate for clients’ rights to client-directed care and active decision-making.
Contribute to quality improvement initiatives and promote a safe working environment.
Foster teamwork and communication across care teams and with other business areas
**Skills and Experiences**:
About You
You’re an empathetic, organised, and proactive leader with a passion for supporting both clients and staff. You bring a strong understanding of aged care best practice and thrive in a role where no two days are the same.
Essential Qualifications & Experience
Certificate III or IV in Aged Care (or equivalent qualification).
Proven experience in community aged care and consumer-directed environments.
Demonstrated leadership experience within a care or support setting.
Strong communication, teamwork, and problem-solving skills.
Proficiency in electronic documentation systems and Microsoft Office.
Current Driver’s Licence, National Police Clearance, First Aid Certificate, and full COVID & Flu vaccinations.
Desirable
Experience working in community aged care.
Understanding of Aged Care Quality Standards.
High level of computer literacy and customer service experience
