Key Responsibilities
- Lead and supervise Care and Support Workers, providing guidance, training, and performance feedback.
- Ensure quality care delivery that meets individual client needs, promotes independence, and upholds Coolibah Care's values.
- Assist in onboarding new clients, helping them transition smoothly to permanent care arrangements.
- Support and collaborate with Care Partners to review and adjust Support Plans.
- Maintain accurate documentation and uphold privacy and confidentiality standards.
- Advocate for clients' rights to client-directed care and active decision-making.
- Contribute to quality improvement initiatives and promote a safe working environment.
Foster teamwork and communication across care teams and with other business areas
About You
You're an empathetic, organised, and proactive leader with a passion for supporting both clients and staff. You bring a strong understanding of aged care best practice and thrive in a role where no two days are the same.
Essential Qualifications & Experience
- Certificate III or IV in Aged Care (or equivalent qualification).
- Proven experience in community aged care and consumer-directed environments.
- Demonstrated leadership experience within a care or support setting.
- Strong communication, teamwork, and problem-solving skills.
- Proficiency in electronic documentation systems and Microsoft Office.
- Current Driver's Licence, National Police Clearance, First Aid Certificate, and full COVID & Flu vaccinations.
Desirable
- Experience working in community aged care.
- Understanding of Aged Care Quality Standards.
High level of computer literacy and customer service experience
