**Role Overview**:
As Support Coordinator you will be responsible for client-facing work (which may be online, by phone, or face to face)to map required services and support family understanding of their NDIS plan. Your role will enhance their experience of life, utilizing NDIS resources from a family-inclusive and person-centered lens.

Tasks include regular involvement with intake, case planning, recording a detailed client history including needs and preferences in our work with them (assessment). Through our online systems, you will keep accurate records of client interactions / outcomes.

You will enjoy a sense of accomplishment in assisting NDIS participants to relieve stress in their lives, improve wellbeing, and establish and maintain supports through their funding, maintaining healthy and positive therapeutic relationships.

**Responsabiites**:

- Engage with individual participants and collaboratively assist with elements of their NDIS goals important to them, with a respectful, phased approach that acknowledges current capacity
- Engage informal and formal support networks, in a family inclusive and collaborative professional approach
- Provide assessment and recommendations to address and mitigate risks and support participant and family capacity and wellbeing. Provide therapeutic support to Clarable clients,developing your professional toolkit.
- Engage Agencies to develop support plans or develop these in-house with the support of the Senior Support Coordinator,based on your understanding of participant and family preferences
- Facilitate Continuity of care and provision of a comprehensive service to all participants
- Utilize intentional, principled practices and observe Clarabel Policies developing your experience of the disability service system, toward a rights-based culture
- Provide client-centered reporting to the NDIA and participate in the review of the package is required

**Work Experience and Qualifications**
- Relevant study in Social Work, Psychology, Occupational Therapy, or Disability Studies / other aligned field.
- A background of 2+ years in the human services or health field or Lived expertise, through health and disability specific needs of themselves or a family member
- Strong Computer literacy Skills and experience using case management software
- Professional development certificates: Mental Health First Aid, Basic First Aid and CPR
- Required Documentation: NDIS worker screening check, Working with Childrens Check, National Police Check, NDIS Orientation Module
- Experience working with clients with dual disability / forensic history
- Experience within disability services or with family carers

**Job Types**: Full-time, Part-time
Part-time hours: 30 - 38 per week

**Salary**: $30.00 - $40.00 per hour
