This is a full-time position. The purpose of the position of Support Coordinator is to liaise with participants to manage and maintain their NDIS plan. The support coordinator will act as the liaison between participants, NDIS, and the service providers, ensuring the inclusion of people with disabilities in the community. We are looking for a Support Coordinator level 3 or Social Worker, Allied Health, Nurse. **Qualifications**: - Previous experience as support coordinator is desirable but not necessary. - Previous NDIS experience is necessary - Respect team members, communicating effectively and demonstrating a commitment to the team. - Holder of a current Driver's License and own vehicle. - Previous experience as support coordinator is desirable but not necessary. - Previous NDIS experience is necessary - Respect team members, communicating effectively and demonstrating a commitment to the team. - Holder of a current Driver's License and own vehicle. **Preferred Skills**: - Establishing relationships with relevant support providers - Encourage and facilitate participant inclusion in community activities - Promote increased independence for participants in line with the participant's NDIS plan - Maintain accurate records of Participant planning activities - Attend and facilitate all Participant support meetings - Continued professional development in accordance with NDIS requirements
