**Description**:
We are seeking a dedicated and compassionate **Case Partner** to join our team, playing a key role in the new **Support at Home Program**. This role focuses on delivering **person-centred care planning**, coordinating services, and building strong relationships with participants and their support networks.

As a Case Partner, you will manage a caseload of participants, ensuring they receive the most appropriate, timely, and effective support aligned with their individual needs and personal goals. You will work closely with participants, families, service providers, and health professionals to ensure services are tailored to each person’s preferences and circumstances.

**Key Responsibilities**
- Manage a caseload of participants within the Support at Home Program.
- Conduct assessments and develop person-centred care plans tailored to individual needs and goals.
- Coordinate and oversee the delivery of care and support services.
- Liaise with participants, families, service providers, and health professionals.
- Monitor, review, and adjust care plans as needs change.
- Ensure compliance with aged care standards and program requirements.
- Maintain accurate and up-to-date client documentation.

**Qualifications**:

- Certificate IV in Aged Care, or a relevant qualification in Nursing, Allied Health, Social Work, or a related field.
- A degree or equivalent experience in aged care is highly preferred.
- Demonstrated experience in case management or service coordination, ideally within aged care or community care.
- Previous experience working with the aged care system is an advantage.
- Current Australian driver’s licence.
- National Police Check and Working With Children Check (or willingness to obtain).
- Current First Aid and CPR certification.
- Commitment to ongoing professional development.

- Problem Solving & Decision Making: Strong ability to assess needs, identify solutions, and make sound decisions in a timely manner.
- Organisational Skills: Able to manage multiple tasks, prioritise effectively, and maintain attention to detail in a fast-paced environment.
