Case Partner

We are seeking a dedicated and compassionate Case Partner to join our team, playing a key role in the new Support at Home Program. This role focuses on delivering person-centred care planning, coordinating services, and building strong relationships with participants and their support networks.

As a Case Partner, you will manage a caseload of participants, ensuring they receive the most appropriate, timely, and effective support aligned with their individual needs and personal goals. You will work closely with participants, families, service providers, and health professionals to ensure services are tailored to each person's preferences and circumstances.

Key Responsibilities

• Manage a caseload of participants within the Support at Home Program.
• Conduct assessments and develop person-centred care plans tailored to individual needs and goals.
• Coordinate and oversee the delivery of care and support services.
• Liaise with participants, families, service providers, and health professionals.
• Monitor, review, and adjust care plans as needs change.
• Ensure compliance with aged care standards and program requirements.
• Maintain accurate and up-to-date client documentation.
