We are seeking someone who has demonstrated experience as **Home Care Package Coordinator** who genuinely enjoys providing case management services for people over the age age of 65 years.

You will be responsible for the coordination, care planning and monitoring of HCP clients to ensure a quality service is provided. You will enjoy building good relationships with clients, their families and other health professionals. You will work closely with our other CHSP and HCP Coordinators and Clinical team. This is a part time position, **based in the Brisbane CBD would suit someone living in the Brisbane North area.** A car is available for in-home visits.

All Aged Care has been operating for 23 years as a quality approved provider of home care services with Head Office located in the Brisbane CBD close to public transport. We are a multi-cultural organisation who pride ourselves on delivering services which exceed client's expectations which enable clients to remain living independently in their own homes.

To be successful in this role you will be self-motivated with a "can do" attitude and have:

- Minimum 1 year Home Care Package Coordinator experience AND a care related qualification such as Case Management skill set or Diploma of Community Services
- Good communication, problem solving and organisational skills
- Demonstrated ability to prioritise and multi-task
- Good computer skills including with care databases, Outlook, MS Word and Excel
- An ability to speak another language is highly regarded

You will need to be a permanent Australian resident, have a National Police Check (or willingness to obtain), open Australian Drivers Licence. You are required to be fully vaccinated - COVID19 and Influenza.

**Job Type**: Part-time
Part-time hours: 20 - 25 per week

**Salary**: $31,678.00 - $40,000.00 per year
