This opportunity is working for All Aged Care, an approved home care provider who has been providing quality in-home services for 23 years in SE QLD. We are a non-profit organisation who pride ourselves on delivering tailored services which exceed client's expectations empowering clients to live their best life in their own home.

This is an excellent opportunity for someone with home care package coordination experience looking for a diverse role as part of close knit team.

**About this Role**:
As a Coordinator you will be working with valued clients, designing tailored care and support plans that meet their individual needs which promote their independence and maximises their abilities.

The successful applicant **will have demonstrated experience** as a Home Care Package Coordinator.

As a team player, you will enjoy working as part of the care team comprising HCP Coordinators, CHSP Coordinators, Registered Nurses and Allied Health.

**To be successful you will need**:

- Experience coordinating Home Care Packages **and**:

- Cert 3 Individual Support (Ageing) or higher
- Ability to develop good professional relationships with clients and their families - you genuinely enjoy making a difference
- Good communication, problem solving and organisational skills
- Ability to work well as part of a multi-disciplinary team
- Australian Federal Police Check
- Current Australian Drivers Licence
- Current First Aid and CPR
- Fully vaccinated against COVID19 and current Influenza

An ability to speak another language is welcomed.

**Salary**: $80,000.00 - $83,000.00 per year
