**About the Acute Psychiatric Unit**
The Acute Psychiatric Unit (APU) forms part of the broader Mental Health Division at Austin Health. It comprises of 19 Acute beds, 5 Eating Disorder beds and a separate wing of 6 Parent Infant Beds.
**About the role**
We are seeking an enthusiastic and highly experienced mental health nurse to undertake a full-time, fixed-term position (June 2025 to May 2026), working 76 hours per fortnight with a monthly ADO. You will join our multidisciplinary team, which includes social workers, occupational therapists, nurses, registrars, consultants, psychologists, and PSAs.
The Transitions Coordinator role is pivotal within the Acute Program Team, ensuring comprehensive, planned, and coordinated admissions and discharges for all consumers. The role is grounded in a systemic approach, and will require strong interpersonal skills to effectively collaborate with internal and external agencies and stakeholders.
A regular day will see you:
- Participate in meetings across the Organisation discussing bed flow and demand
- Participate in Statewide Mental Health Meetings reporting on Austin Health bed demand
- Ensure that Mental Health Consumers are admitted in a timely manner and that discharges are planned with appropriate follow up in the community
- Ensuring APU meets its monthly KPI's
- As part of the APU leadership team, attending operational and clinical meetings.
**About you**
As Transition Coordinator it is essential that you have excellent communication skills, with a strong commitment to developing strong connections across Austin Health. You will need to bring energy and drive to your work, with a passion for ensuring the Quest principles are applied.
To be successful in this role you’ll bring with you:
- Registered Nurse with current AHPRA registration
- Post-graduate qualification in psychiatric/mental health nursing and five years post graduate mental health nursing experience
- The ability to balance the requirements of the organisation with the clinical and operational requirements of APU
- Be able to collaborate within the APU multidisciplinary team to ensure that excellent patient care is delivered to all consumers and families
- Emotional maturity and excellent communication skills.
