About the SchoolHester Hornbrook Academy is an independent, Special Assistance School delivering flexible, inclusive education for young people aged 15–25 across Greater Melbourne.

Position PurposeThe Pathway and Transition Co-ordinator supports students and alumni to achieve positive post-school outcomes through transition planning, alumni engagement, and tailored support. As part of the CAPE Team, this role coordinates the Alumni Program, manages School-Based Apprenticeships and Traineeships, and provides ongoing outreach and case management to former students. The position ensures every student has access to meaningful pathways and is tracked for five years after leaving the academy, using these insights to strengthen future programs. Working collaboratively with staff, families, and community partners, the Pathway and Transition Co-ordinator plays a vital role in empowering young people to succeed beyond school.

Location:This role will be based across multiple Melbourne campuses including CBD, South Melbourne, Sunshine, Werribee, and new development sites.

Key Selection Criteria(Abridged; full details in position description)

    • Qualification in Youth Work, Social Work, or Community Development (or equivalent experience)
  • Experience supporting young people into employment, education, or community participation
  • Working with Children Check (Employee), National Police Check, and right to work in Australia
  • Current Victorian Driver's LicenceEssential Requirements
  • National Police Check and Working with Children Check (Employee)
  • Proof of identity and right to work in Australia
  • Current Victorian Driver's Licence
