An exciting opportunity exists for a dedicated Transition Discharge Coordinator to join our team at Western Health's Mental Health Service.

In this role, you will play a crucial part in managing and supporting clinical activities related to patient flow at Sunshine Mental Health and Wellbeing Centre, Unit 3.

Your responsibilities will include overseeing access and flow, establishing clinical standards at transitional points of care, ensuring effective resource utilisation, and providing clinical leadership and guidance to the multidisciplinary team. This role requires significant liaison and negotiation with senior clinical and non-clinical stakeholders, as well as a well-developed working knowledge of mental health services and external agencies.

**To be Successful in this Role, you will have**:

- Demonstrated ability to effectively manage and coordinate clinical activities related to patient flow in acute mental health settings.
- Proven experience in providing clinical leadership and guidance to multidisciplinary teams.
- Strong problem-solving skills with the ability to negotiate and communicate effectively with stakeholders.
- Commitment to continuous professional development and adherence to evidence-based practices.
- A compassionate and empathetic approach, with the ability to engage and support mental health consumers from diverse backgrounds.
