**Your Role**

As a QCSS team member you'll be looking after the most important people in our organisation - our clients.

Your tasks will include:

- Undertaking telephone assessments and referrals
- Screen and determining eligibility, and assessing required needs with new or current service users, carers, family and other persons as appropriate
- Providing information and assistance in regards to Ozcare community based services
- Co-ordinate care plans for new and existing clients
- Promote a high level of customer service to internal and external customers
- Respond to telephone calls promptly and appropriately
- Complete all required documentation for contact centre intake and assessment protocols

**Role Requirements**
- Understanding of Queensland Community Support Scheme would be beneficial
- Ability to use motivational interviewing techniques to gather screening and assessment data in order to determine service user eligibility for access to QCSS services
- Relevant qualifications in community services and/or relevant experience in a health related field
- Excellent computer skills
- Demonstrated sound written and verbal communication
- Ability to work collaboratively with all members of the QCSS Access Point team
