In this diverse role, you'll be at the forefront of delivering exceptional care coordination and comprehensive support services to clients who are experiencing alcohol and other drug dependency and other complex support needs. As a Team Leader, you will guide and inspire your team, in making a meaningful difference in the lives of those you assist.
Your Responsibility- Assist the Manager with developing and implementing the service model, ensuring alignment with Vinnies Services' policies and the AOD Model of Care.
- Manage and support Care Coordinators, including their professional development, performance, and evidence-based practice.
- Oversee care reviews, intake, and assessment processes, including risk management and coordinator allocation.
- Ensure a safe environment for residents, staff, and visitors, and maintain compliance with policies and WHS regulations.
- Coordinate care provision, engage with referring agencies, and provide direct support according to the AOD Model of Care.
- Act as a key Care Coordinator, support other Care Coordinators, and conduct comprehensive assessments and care planning.
- Collaborate with LHD & NGO staff, develop referral pathways & provide support in employment, housing, and social engagement.
- Support evidence-based treatment, conduct regular appointments, and provide ad hoc support as needed.
- Facilitate educational groups when necessary and engage in case reviews and stakeholder meetings.
- Maintain accurate client records, conduct assessments, support family engagement, and adhere to legislative requirements.
- Participate in staff meetings, training, quality improvement activities, and performance appraisals.
- Relevant tertiary qualification/significant relevant experience.
- Extensive experience in managing case loads and planning for complex needs (e.g., co-occurring alcohol and other drugs, homelessness, mental health, criminal justice).
- Proven ability to manage and lead small teams.
- Strong networking and community care coordination skills in the relevant region.
- Knowledge of and ability to access relevant community resources.
- Computer literacy and experience with data collection.
- Unrestricted NSW driver's license.
