**The Company**

Trilogy Care is an Approved Provider of the Federal Government’s Home Care Package Program. We are an enthusiastic and dynamic organisation, dedicated to supporting elderly Australians to remain supported and independent in their own homes.

**The Role**

We are currently looking for an Administration Assistant to join our Care Management Team based out of our Fortitude Valley office. Working with a tight-knit team, you will be responsible for the delivery of administrative assistance to the Care Management Team and undertake consumer liaison as required.

We are seeking applicants with highly developed customer service or administrative experience who will champion this consumer-directed care with our clients. Experience in a health or aged care setting is not necessary.

**Duties**
- Documenting feedback, complaints, and concerns,
- Scheduling of meetings,
- Setting up accounts with providers - meals, services, pharmaceuticals, etc.
- Lodging incidents
- Lodging leave

Provide administrative assistance to the Care Management Team, including:

- Savings ACAT & NSAF assessment documents to the Consumer’s file
- Sending Care Plans and Budgets to the Consumer for review
- Releasing on-hold bills
- Preparing evaluations and annual reviews, making sure all information is up to date and correct in accordance with My Aged Care
- Sending referrals on behalf of Care Partners to OTs, allied health professionals, and other providers

**Skills & Attributes**
- Demonstrates highly developed administrative skills
- Application of Ring Central, SalesIQ, and Zoho CRM (desired but not essential)
- Works independently and as a member of a team and uses initiative to achieve results
- Continually takes the opportunity to develop personal and professional skills and knowledge
- Excellent communication and relationship skills to deal effectively with Consumers, colleagues, suppliers, and stakeholders
- Builds and maintains productive relationships through professional and ethical interactions
- Proactively approaches and resolves conflict and seeks support/advice where necessary

**Mandatory Requirements**
- Current Federal Police Clearance (not more than 12 months old) and international Police Check if you have lived overseas in the last 10 years.
- Evidence of COVID Vaccination (industry requirement)
- Evidence of Australian work rights (Passport, Birth Certificate etc).

**Role Type**:

**Pay Rate**:

- 60000 AUD - 60001 AUD (Annum)
