**The Company**

Trilogy Care is an Approved Provider of the Federal Government’s Home Care Package Program. We are an enthusiastic and dynamic organisation, dedicated to supporting elderly Australians to remain supported and independent in their own homes.

**Role**

Our Care Team manage a personal portfolio of clients in line with the recipient’s individual goals and needs. We develop long-lasting relationships and are a trusted supporter of our consumers on their home care journey.

We are seeking applicants with customer service or case management experience who will champion this consumer-directed care with our clients, working with them to develop their care plan and care budgets, and providing ongoing support.

Experience in a health or aged care setting is not necessary.

**Duties**
- Holding meetings with recipients and/or their representatives to determine their care and support goals.
- Documenting the care plan and budget as developed with the recipient.
- Being the trusted point of contact for all aspects of the recipient’s Home Care Package.
- Coordinating and performing periodic client care plan reviews and other related activities.
- Administration of information, documentation and systems relating to care management.
- Ongoing communication with clients and their representatives, stakeholders, service providers and government agencies.

**Mandatory Requirements**
- Current Federal Police Clearance (not more than 12-months-old) and international Police Check if you have lived overseas in the last 10 years.
- Evidence of COVID Vaccination (industry requirement)
- Evidence of Australian work rights (Passport, Birth Certificate etc).

**Salary Package**
- $60,000 per annum, plus 10.5% superannuation
- Annual bonuses based on service quality & efficiency
- On-site parking one day per week

**Role Type**:

- In-house - Permanent - Full-time - Entry level

**Pay Rate**:

- 60000 AUD - 65001 AUD (Annum)
