**The Role**
Our Care Team manages a personal portfolio of clients in line with the recipient’s individual goals and needs. We develop long-lasting relationships and are trusted partners in our consumers' home care journey.
We are seeking applicants with clinical, nursing, or health industry experience who will champion this consumer-directed care with our consumers, working with them to develop their clinical care plan and care budgets, and providing ongoing support.
**Duties**
- Hold meetings with recipients with complex clinical care needs and/or their representatives to determine their care and support goals
- Document the care plan, clinical and risk plan, and budget as developed with the recipient
- Be the trusted point of contact for all aspects of the recipient’s Home Care Package
- Coordinate and perform periodic client care plan reviews and other related activities
- Administrate information, documentation, and systems relating to care management
**Professional Skills**
- Completed or currently completing a qualification in Nursing or a related health-based role
- Relevant understanding and experience in the delivery of health services
- Confident and skilled in supporting and working with clients, preferably in a telehealth setting
- Experience using online care management systems (desirable but not essential)
- Client-focused care and case management experience (desirable but not essential)
- A practical understanding of the Aged Care sector and Quality Standards (desirable but not essential)

- Well-organised, able to prioritise, and meet competing deadlines
- Excellent attention to detail, able to problem solve and make decisions
- Enjoys working in a multi-disciplinary team
**Salary Package**
- Starting salary commensurate with qualificationss and experience ($65,000 - $80,000 per annum) plus 10.5% superannuation
- Annual bonuses of up to 20% of base salary, based on service quality & efficiency

**Mandatory Requirements**
- Current Federal Police Clearance (not more than 12 months old) and international Police Check if you have lived overseas in the last 10 years
- Evidence of COVID Vaccination (industry requirement)
- Evidence of Australian work rights (Passport, Birth Certificate, etc)
