Carepro Disability Services is a rapidly growing registered NDIS Service Provider with offices based in Melbourne and Perth. Our company has been supporting the disability sector since 2017 and we believe that exceptional customer service starts with employees who love what they do. Due to our continued and exciting growth we are now seeking an experienced Support Coordinator to join our team in the Broadmeadow's area.
Our staff are empathetic by nature and have a genuine interest in supporting participants to live independently and achieve their goals. We take pride in assisting those most vulnerable in our community.
**About The Role**
This role is part-time offering flexible working arrangements. As a Support Coordinator you will assist participants to navigate and implement their NDIS plan's by working alongside service providers and allied health professionals. You will manage your own caseload of NDIS participants, providing them with guidance to develop goals and demonstrate a person-centered approach to all aspects of the role by supporting clients to achieve their goals in line with their NDIS Plans. This includes educating and empowering participants to exercise choice and control while learning and building skills. We are looking for someone who can take on an existing caseload and hit the ground running.
**About You**
To be successful in the role, it is a requirement that you are an experienced professional who has previous Support Coordination experience working with clients with disability. You possess a deep understanding of the NDIS and knowledge of the rules relating to participant planning and reasonable and necessary supports. You show demonstrated ability of working within a client-focused environment, to build client capacity and to provide choice and control to participants. You have excellent communication skills and the ability to evaluate services and funding and continually seek growth opportunities for clients. You are empathetic of the challenges faced by vulnerable people and those supporting them. You are goal orientated with an ability to meet KPI's and billable hour targets and are proficient in Microsoft office suites with CRM system experience.
**Responsibilities**
- Conduct assessments of individuals' needs and develop individualized support plans in collaboration with the individual, their family, and other service providers.
- Coordinate and monitor the delivery of services to ensure they meet the individual's needs and goals.
- Advocate on behalf of the individual to ensure they receive appropriate services and supports.
- Maintain accurate records and documentation of all services and interactions.
- Provide regular updates and reports to the individual, their family, and other stakeholders.
- Stay up-to-date on current policies and regulations related to disability services.
- Manage and plan budgets

You will be paid in accordance with the SCHADS Award Level 3 pay point 1. You will be required to use your own vehicle for work purposes and will receive a kilometer allowance for all kilometers travelled. The position offers the potential for full time hours in the future.
