**About Us**
Greater Northern Community Services (GNCS) is a values-driven NDIS provider supporting adolescents and adults with complex disabilities to build fulfilling, empowered lives. We work across Melbourne, specialising in psychosocial disability, trauma-informed practice, and complex support coordination.
**The Role**
We are seeking an experienced **Support Coordinator / Psychosocial Recovery Coach** to deliver **KPI-aligned support** across a varied caseload of participants requiring Level 2 and Level 3 coordination. You will work with high-risk, forensic, psychosocial, or behaviourally complex participants and navigate multi-agency systems including mental health, housing, justice, and clinical teams.
As a **Support Coordinator / Psychosocial Recovery Coach**, you will work alongside experienced, passionate professionals, helping participants to build capacity, navigate systems, and achieve their goals. Our leadership team supports you with regular check-ins, guidance, and the autonomy to manage your own caseload while remaining connected to a collaborative team environment.
Our flexible hybrid model includes remote work, field visits, and in-person supervision days at our Melbourne base. You will be supported by a collaborative and caring team with strong clinical oversight and regular reflective practice.
**Key Responsibilities**:
- Play a vital role in creating safe, empowering, and person-centred experiences
- Conduct assessments and develop tailored support plans in collaboration with participants
- Assist participants to understand, navigate, and maximise their NDIS funding
- Coordinate a wide range of services including mental health, AOD, justice, housing, and safeguarding
- Facilitate hospital and justice transitions with a focus on complex needs and risk mitigation
- Lead case conferences and manage multidisciplinary input
- Advocate strongly for participant rights, autonomy, and long-term outcomes
- Monitor and report on goal progress, making plan adjustments where required
- Maintain accurate, up-to-date case notes and ensure compliance with all NDIS and safeguarding standards
**Work Hours**:
- Hours based on participant plan funding - typically 10-25 hours/week
- Flexible scheduling - remote and field work options available
**Remuneration**
- You will be paid fortnightly.
- Rates will be competitive and based on experience and qualifications, in line with industry standards (+ Superannuation). Specific rates will be discussed with successful applicants.
**Eligibility Criteria**
- **Must have experience** delivering **Support Coordination Level 2 / Psychosocial Recovery Coach**:
- Relevant tertiary qualification in Social Work, Psychology, Mental Health, Community Services, or equivalent
- Proven ability to manage high-risk, high-complexity participants
- Exceptional communication, stakeholder engagement, and crisis response skills
- Strong understanding of the NDIS, restrictive practices, and safeguarding
- Current **NDIS Worker Screening**, **WWCC**, **First Aid (HLTAID011)** and **CPR (HLTAID009)**:
- **National Police Check** (within the last 12 months)
- **Full Driver’s Licence** and reliable vehicle
- Must have **the right to work in Australia**:
- Must be confident working independently and remotely
**Job Types**: Part-time, Casual
Pay: $45.00 - $50.00 per hour
Schedule:
- Flexible weekday availability
- Crisis management support as needed
- Remote work available with in-person meetings as required
