**Your new role**
You will be a part of a newly formed team that provides specialist support to Australian's with disability who are living in, or at risk of permanently entering aged care facilities under the age of 65. You will liaise with stakeholders and be an integral link between participants, the agency and hospital staff. You will be leading and managing a team to perform to a high standard and deliver quality outcomes. You will follow all legislation, policies, procedures along with standards and performance metrics. You will identify any opportunity for process improvement and innovation within the team to ensure smooth deliveries for participants. There are two positions available. We are seeking an enthusiastic Team Leader to work in Box Hill or Sunshine.

**What you'll need to succeed**
You will have proven experience in leading a team to drive successful outcomes, and if it is within a government sector then it is highly regarded. You will take initiative and lead the team by example. By coaching and mentoring your team you will provide reports on performance and data and implement new strategies to drive effective outcomes. Clear written and verbal communication skills is essential as you may be speaking with participants who have a disability. Empathy, patience, adaptability and resilience are also qualities you will possess.
