We are currently seeking an **experienced, proactive, and compassionate Support Coordinator** to work with **Level 2 and Level 3 NDIS participants**. This role is ideal for someone who thrives in a dynamic environment, is confident in complex case management, and is passionate about advocating for participants' needs.

As an experienced Support Coordinator, you will be responsible for working closely with participants with **complex needs**, helping them to understand and implement their NDIS plans, coordinate supports, and achieve their goals.

You will be the key point of contact, working across various stakeholders, including service providers, allied health professionals, housing providers, and clinical teams.

**Your Key Responsibilities**
- Provide Support Coordination for NDIS participants with Level 2 (Support Coordination) and Level 3 (Specialist Support Coordination) plans
- Assist participants in understanding and activating their NDIS plans
- Coordinate a range of supports, including mainstream, informal, and funded supports
- Manage complex participant cases, including crisis response and risk planning
- Build strong relationships with participants, families, carers, and providers.
- Monitor progress toward goals and adjust support as needed
- Maintain accurate and up-to-date documentation and reporting in line with NDIS Quality and Safeguards standards

**What You’ll Bring**
- **Minimum 3 years experience** in NDIS Support Coordination (with experience managing Level 2 & 3 participants)
- Proven experience supporting participants with **complex needs** (e.g. psychosocial disability, dual diagnosis, or high-risk environments)
- Strong knowledge of the **NDIS framework**, funding categories, and service delivery models
- Relevant qualification in **Social Work, Psychology, Allied Health, or Community Services**:

- High-level communication, negotiation, and advocacy skills
- Ability to manage competing priorities and work autonomously
- Current NDIS Worker Screening Check, Working with Children Check, and Police Check
- Valid Driver’s License and reliable vehicle

Pay: $85,000.00 - $100,000.00 per year
