**Help Along Pty Ltd** is seeking some experienced **Disability Support Workers (In-Home Support)** to provide Person-Centred support to NDIS participants across various suburbs in Brisbane North to the Sunshine Coast region.

This position is for an experienced Support Worker on a casual (Leading to permanent part-time) basis to provide ongoing support to our NDIS participants.

- **Competitive rate of $31-$81.00** (Depending on days and times worked and level of experience)

Duties may include anything from providing personal care to assisting the client in accessing the community. Previous experience supporting People with disabilities or Mental Health issues is preferable. You must have strong communication skills with the ability to build and maintain a positive relationship with all the stakeholders.

**Minimum Requirements**
- Certificate III or above in Disability or other similar disciplines
- Police Check
- Working with Children Check (Known as Blue Card)
- NDIS Clearance Certificate or Yellow Card
- Certificate of NDIS Orientation Module
- Covid-19 Vaccine Certificate (Fully vaccinated)
- 2 x References
- Current QLD Driver’s License
- Comprehensive Car Insurance

**Key Responsibilities**
- Follow individual support plans, behavior support plans, and health care plans.
- Participate in community activities within a Supported Independent Living environment.
- Maintain and monitor a client’s support plan whilst modeling a person-centered approach.
- Identify and support a range of learning opportunities for our clients.
- Observe and document an individual’s progress and any changes through detailed note-keeping

**Job Type**: Casual

**Salary**: $31.00 - $81.00 per hour

- First Aid Certification (required)
