We are seeking a dedicated and experienced Service Coordinator to join our growing team. If you're passionate about delivering high-quality, person-centred care and leading teams with purpose, this role is for you.

Key Responsibilities

Participant Support & Quality Care
- Oversee client onboarding, including intake assessments, documentation, and service planning. Provide clinical on-call support to manage urgent situations and guide staff in high-risk scenarios. Monitor and ensure service delivery aligns with NDIS Practice Standards and each participant’s goals.

Team & Staff Leadership
- Offer regular guidance and support to Team Leaders with clear expectations and performance feedback. Lead staff meetings (weekly/fortnightly) to discuss service delivery, compliance updates, and training.

Stakeholder Engagement
- Maintain strong communication with participants’ families, support coordinators, health professionals, and agencies. Build positive stakeholder relationships to facilitate smooth service coordination and escalation management. Represent the organisation in stakeholder meetings to ensure satisfaction and feedback integration.

Compliance & Reporting
- Collaborate with participants and stakeholders to develop and review individualised support plans. Handle incidents, feedback, and complaints with professionalism and timely responses. Monitor notes, behaviours, restrictive practices, and ensure accurate reporting to the NDIS Commission. Conduct regular SIL house visits to assess care quality and maintain strong on-ground connections. Ensure all client files are audit-ready and compliant with regulatory standards.

What We're Looking For
- Experience in the disability, health, or community services sector. Strong leadership and communication skills. In-depth understanding of the NDIS and its compliance requirements. Ability to manage complex service environments and support staff through challenges. Passion for participant-centred care and continuous improvement.
