Empowered Community Health & Wellbeing (ECHW) is more than a service—we are a movement for empowerment. We guide individuals with disabilities and mental health challenges towards independence, self-advocacy, and an improved quality of life. We are seeking a Support Coordinator to join our passionate team and help clients navigate the NDIS system with confidence and clarity.

About the Role:
As a Support Coordinator, you will be the trusted guide for NDIS participants, helping them maximise their funding, access the right services, and build the skills needed to thrive. You’ll work alongside a team of experienced developmental educators, counsellors, and mentors to provide holistic, person-centred support.

Your Responsibilities:

- Help participants understand and implement their NDIS plan to achieve their goals.
- Break down barriers to accessing services, ensuring clients receive the right support.
- Build strong relationships with clients, families, and service providers.
- Assist with referrals, service agreements, and progress monitoring.
- Advocate for client rights, independence, and self-advocacy.
- Ensure compliance with NDIS guidelines and ECHW values.

- Excellent problem-solving and communication skills.
- Ability to work independently and collaboratively.
- Experience in support coordination, case management, or allied health preferred.
- Lived experience in disability or mental health is highly valued.

Requirements of the Role:

- **Relevant Qualification** - Bachelor’s degree or diploma in Social Work, Psychology, Disability Studies, Community Services, or a related field.
- Strong knowledge of the NDIS and disability support sector.
- Minimum 2+ years’ experience in support coordination, case management, or allied health.
- Experience working with individuals with disabilities or mental health challenges.
- Ability to navigate complex systems and assist clients in accessing the right services.
- Strong problem-solving and decision-making skills.
- Excellent written and verbal communication skills.
- Ability to work independently and within a team.
- Strong relationship-building and stakeholder engagement skills.
- Commitment to empowering clients towards independence and self-advocacy.
- High level of organisational and time management skills.
- Understanding of compliance and regulatory requirements.
- Current Working with Children Check, NDIS Worker Screening Check, and National Police Check.
- Valid driver’s licence and access to a reliable vehicle.

Pay: $35.50 - $43.50 per hour
