All Aged Care is a well established provider of home care aged care services and has a track record for provision of quality, compliant care and services in SE QLD. We are seeking someone who has demonstrated experience as **either a CHSP Coordinator or Home Care Package Coordinator **who genuinely enjoys providing care management services for people over the age of 65 years.

You will be responsible for the coordination, care planning and monitoring of HCP clients to ensure a quality service is provided. You will enjoy building good relationships with clients, their families, other health professionals. You will work closely with our other Coordinators, Registered Nurses and Allied Health teams.

This is a **PART TIME **position (Monday to Friday - no oncall), based in the Brisbane CBD close to public transport with company cars available for in-home visits. There is the opportunity for professional growth within this organisation.

**To be successful in this role you will have**:

- A minimum of 1 years experience as a HCP Coordinator or CHSP Coordinator
- Case Management Skills set or Diploma of Community Services or equivalent qualification
- Ability to prioritise and multi-task
- Be Self-motivated with a "can do" attitude
- Excellent communication, problem solving and organisational skills
- Demonstrated ability to work well as part of a team as well as autonomously
- Good computer skills including with care databases, word, excel, outlook

If you speak another language that will be very welcomed.

You will need to be a permanent Australian resident, have a National Police Check (or willingness to obtain), open Australian Drivers Licence. You are required to be fully vaccinated - COVID19 and Influenza.

**Job Type**: Part-time
Part-time hours: 20-25 per week

**Salary**: $31,678.00 - $40,000.00 per year
