We are looking for a motivated, dynamic, and detail-oriented Care Partner to join our team at our Spring Hill office. We are seeking someone who has demonstrated experience as a **Home Care Package Coordinator** and genuinely enjoys providing care management services for people over the age of 65 years.

**The Role**
As a Care Partner, you'll play a crucial role **coordinating both CHSP and HCP (SAH) clients**, including onboarding new clients and providing ongoing care management, care planning and monitoring to ensure a high-quality service. You will take the lead with organising reassessments and supporting clients **transitioning from CHSP to HCP/Support at Home**.

With a manageable caseload, you will thrive on building strong professional relationships with both your clients and their families and the Care Workers providing their services. You'll **support and mentor care workers, oversee service continuity and coordinate in-home visits.**

**About You**

Ideally, you'll bring experience in both CHSP and Home Care Packages and have some knowledge of the new Support at Home program.

You will enjoy mentoring and supporting care workers and working as part of a team, collaborating with others, sharing ideas and working towards a common goal - providing exceptional care and services to our valued clients. You will be well-supported by the organisation and the team of Care Partners, Registered Nurses and Allied Health professionals.

**What You'll Bring**
- Experience in community aged care with demonstrated experience and knowledge of either Home Care Packages or CHSP programs with some knowledge of the Support at Home Program (training provided)
- Strong organisational skills including problem solving, prioritisation and multitasking
- Excellent communication and interpersonal skills with a collaborative team spirit and "can do" attitude
- Proficiency in MS Office and care management databases
- Current open Australian Drivers Licence with ability to drive to clients homes
- Current First Aid and CPR certification
- Fully vaccinated - COVID19 and current Influenza
