We are currently seeking a dedicated and compassionate NDIS Case Manager to join our team. As an NDIS Case Manager, you will be responsible for coordinating services and supports for clients with disabilities who are eligible for the National Disability Insurance Scheme. Your primary role will involve conducting assessments, developing care plans, and advocating for clients to access the appropriate services and resources they need to achieve their goals and improve their quality of life.

**Key Responsibilities**:

- Conduct assessments to determine client needs, goals, and preferences
- Develop and implement individualized care plans in collaboration with clients and their support networks
- Coordinate a range of services based on individual needs
- Monitor and review client progress regularly and adjust care plans as needed
- Assist in employment and client sign ups
- Maintain accurate and up-to-date documentation of client assessments, care plans, and progress notes
- Keep abreast of NDIS guidelines, policies, and procedures to ensure compliance with regulations
- Provide support and guidance to clients and their families to navigate the NDIS system and access available services
- Must be willing to travel between Logan and Gold Coast.

**Qualifications and Skills**:

- Minimum 2 years experiance in a support field.
- Proven experience working with individuals with disabilities and knowledge of the NDIS framework
- Strong communication, interpersonal, and advocacy skills
- Ability to work independently and as part of a team
- Excellent organizational and time management skills
- Valid driver's license and access to a reliable vehicle for travel to client meetings
- Knowledge of community resources and services available to individuals with disabilities
- Cultural sensitivity and awareness of the diverse needs and backgrounds of clients
- Blue Card and Yellow Card

**Bonuses**:

**Job Type**: Part-time

**Salary**: $33.00 per hour

Expected hours: No less than 18 per week
