Job Description:

We are seeking an experienced Case Manager to join our team. As a Case Manager, you will play a vital role in the management of our clients and their supports.

Key responsibilities include managing a caseload of clients, ensuring staff are trained and allied health teams are engaged. You will also be working with clients towards their goals and attending multi-disciplinary meetings.

The role is a mix of being in the office and being out in the community visiting clients, training support teams and onboarding new clients. Company vehicles are available for use.

Essential Skills and Qualifications:

  • Previous case management and/or supervisory experience
  • Support Coordinator experience highly desirable
  • Ability to work collaboratively in a multidisciplinary team
  • Highly developed verbal and written communication skills
  • Confidence in managing time effectively, including autonomy and teamwork
  • IT savvy
  • Blue card and NDIS Workers Screening Clearance or willingness to obtain
  • Availability to participate in an on-call roster (paid allowance)
