Our Client is a not for profit community organisation who works to enhance the independence and lifestyle of people living with a brain injury, neurological condition or disability.
They are a registered NDIS provider offering a range of services and opportunities to achieve optimal health, wellbeing and lifestyle in the community.
The organisation began over 30 years ago when a group of people on the Gold Coast, each with a family member who Acquired Brain Injury (ABI), recognized the need for specialized care and support for their loved ones.
**The Role**:
We are recruiting an experienced NDIS Support Coordinator to join a small, supportive Not-For-Profit based in Molendinar, on the Gold Coast.
This is a permanent, full time role that offers work from home flexibility _(provided KPI's are being met)_.

This role will oversee the engagement and implementation of the organisations suite and other independent registered supports and services under NDIS.
The key aspect of the role is the management and supervision of a caseload of approximately 30 NDIS clients.
The Support Coordinator provides individualised, flexible support to people living with an Acquired Brain Injury.
Person Centered support planning is the foundation to effective support and actively involves clients, their support networks and other key stakeholders at all stages of the cycle to enhance ‘quality of life’ for each service users.
**Key Responsibilities**:
- Drafting and monitoring service agreements relevant to participants NDIS requirements, including allocated funding
- Engaging and coordinating service providers on behalf of clients and linking clients to relevant providers and services in accordance with individual NDIS plans
- Assisting clients and their formal supports (families/carers/guardians) to understand their NDIS plan and associated processes
- Identify services options and assist clients within the parameters of NDIS funding allocation
- Work with clients to help build their own capacity and independence using a coaching approach
- Maintain accurate case management notes (CTARS) to record progress, agreed outcomes and actions
- Track and accurately record client’s hours of services and support (claims)
**To Be Successful**:
- Formal qualifications relevant to this position in Disability, Community Services, Social Welfare or a related discipline
- Experience, knowledge and understanding of the National Disability Insurance Scheme
- Proven experience in a similar role in the disability, health or community services sectors
- Ability to meet assigned Key Performance Indicators (KPIs) through the client billing process
- Knowledge of client-centered practices with awareness and professionalism to form positive relationships within key stakeholders
- Strong communication and conflict resolution skills, administrative skills and time management
**Additional Requirements**:
- Current QLD Drivers License
- Current Blue Card and NDIS Worker Clearance
- Fully vaccinated against COVID-19
**Salary and Benefits**:
