**The Role**

Danny Met Sally Support Coordination is growing and we are looking for a passionate and empathetic NDIS Support Coordinator to join our journey and purpose of delivering premium quality services to our Participants

Key Responsibilities:

- Building a genuine rapport and maintaining relationships with each Participant
- Assist Participants to understand and utilise their NDIS plans
- Connect Participants with suitable and necessary providers, while expanding their community and mainstream services
- Assist Participants in building their skills and independence
- Liaising with Participant and stakeholders to achieve best outcomes for the Participant
- Managing an existing case load and connecting new Participants with our Support Coordination services
- Networking with NDIS Service Providers to ensure Participants have an adequate choice of quality providers

**About You**
- Great understanding and knowledge of the NDIS, including: funding categories / allocations, processes, regulations and reviews
- Minimum 12 months’ experience as a NDIS Support Coordinator
- Empathetic and understanding approach
- Experience with writing case notes and detailed documentation
- NDIS Worker Screening Check
- Attention to detail
- Clear and transparent communication
- Genuine desire to help others and achieve quality outcomes for Participants
- Drivers’ License
- Professional Approach to Service Provision
