**About the Role**:
The Support Coordinator position will provide responsive, individualised services and support for NDIS participants and their families. They are to deliver a person-centred approach and provide empowerment, choice, control, and community inclusion of people with disabilities and their families that is consistent with their NDIS plan.
The Support Coordinator will report to the Service Manager.
Experience of at least 1-4 years in a Support Coordinator Role or experience in disability & mental health.
**Responsibilities**:
- Coordinate services as defined in the participant's NDIS plan, using a strengths-based and capacity building approach.
- Conduct a needs assessment with the participant, based on their NDIS goals and the level of service defined in the NDIS plan.
- Provide to the NDIA timely and relevant progress reports reflecting progress towards achieving plan goals, and recommendations about the appropriate type of support required in the next plan and reasons for this.
- Provide person-centred, and where appropriate, family-centred support based on developing skills, competencies, and resources to enable the participant and their family to promote the participant's wellbeing and development and the family's capacity to be self-managing.
- Provide services and maintain written program documentation consistent with the Disability Act and the NDIS Act.
- Comply with mandatory reporting requirements, e.g. child safe policies, and abuse and neglect of adults..
- Establish and maintain effective links with the NDIS, and other provider services.
**Essential Criteria**:
- Creative attitude with a ‘think outside the box’ mindset.
- Ability to work effectively as part of an established team.
- Experience in the disability sector.
- Strong organisational and multitasking skills.
- Excellent verbal and written communication skills.
- Current drivers’ licence.
- NDIS Worker Orientation Certificate (or willing to obtain)
- NDIS Worker Screening (or willing to obtain)
- Working with children check (or willing to obtain)
- Current First Aid Certificate (or willing to obtain)
- A working understanding of relevant legislation and NDIS procedural guidelines.
- While not a pre-requisite, qualifications in a disability related field are considered.
- Experience in NDIS Support Coordination.
