Essentially Your Choice is looking for an experienced NDIS Support Coordinator to assist people living with a disability to achieve their NDIS plan goals. The Support Coordinator will work closely with the participant (and support network) to build their capacity and assist them to coordinate their informal supports, mainstream supports and funded supports.
- Meet with participants to identify and prioritise individual goals and initiate support requirements
- Assist participants to understand how to use their NDIS plan effectively to achieve their goals
- Coach participants (and informal supports) to build their understanding on how they can utilise and implement their NDIS plans
- Promote independent decision making and individual choice and control 
- Source and liaise with relevant service providers
- Build and maintain proactive client-focused relationships with stakeholders, including mainstream supports and funded support services
- Monitor plan outcomes, budgets and expenditure
- Support participant to resolve any challenges that arise during plan implementation or in any crisis as they arise
- Maintain accurate records and note keeping
- Prepare formal written documents as required, including NDIS End of Plan reports
- **The Successful Applicant will have**:

- Previous experience working as a Support Coordinator, case management position or working within the mental health and disability industry
- Qualifications in community services, disability, mental health or allied health or willingness to obtain
- A strong knowledge of the NDIS including an understanding of NDIS plans, operational guidelines and frameworks
- Knowledge of relevant service providers in the area, including mainstream and community services
- Excellent computer skills, including use of Microsoft Office programs and client management systems
- Effective time management with strong skills in responding and adapting to competing priorities

**Job Types**: Full-time, Part-time, Permanent
Part-time hours: 30-38 per week

**Salary**: $33.00 - $40.00 per hour

**Experience**:

- Community & Human Services Occupations: 1 year (preferred)

Licence/Certification:

- Working with Children Check (preferred)
- Driver Licence (preferred)

- Australia (preferred)
