
Coordinator of Supports

**Key Responsibilities**:
- **Assessing Needs and Goals**: Collaborate with participants to identify their unique needs and aspirations, ensuring customised support.
- **Developing Support Plans**: Create comprehensive support plans in partnership with participants, outlining necessary services and strategies for goal achievement.
- **Coordinating Services and Support**: Facilitate arrangements for various services, connect participants with providers, and manage schedules to ensure effective communication among all parties.
- **Monitoring Progress and Outcomes**: Regularly review and assess participants progress toward their goals, making adjustments to support as needed.
- **Advocating for the Individual**: Act as a strong advocate for participants, ensuring their rights and preferences are respected within the disability services system.
- **Linking with Community Resources**: Assist participants in accessing community resources, including social activities, educational programs, employment opportunities, and healthcare services to promote integration and independence.
- **Providing Guidance and Information**: Offer valuable information and guidance to participants and their families, empowering them to make informed decisions about their support options.
**Qualifications**:
- **Education and Experience**: Relevant qualifications in the community services industry, with a strong background in disability services.
- **Strong Communication and Interpersonal Skills**: Exceptional ability to build rapport, actively listen, and communicate effectively with participants, families, and service providers.
- **Knowledge of Disability Services**: In-depth understanding of the disability services landscape, including funding options and relevant policies.
- **Problem-Solving and Decision-Making Skills**: Proven ability to identify challenges, develop solutions, and make informed decisions that prioritize participants' best interests.
- **Cultural Sensitivity and Inclusivity**: Commitment to cultural sensitivity, respecting diverse backgrounds and ensuring inclusivity in all aspects of support.
**Job Type**: Subcontract
Pay: $60.00 - $70.00 per hour
