**Service Coordinator - Toowoomba**

Are you a compassionate professional ready to lead in disability services? Join our team as a **Service Coordinator** at our Supported Independent Living (SIL) villas in Toowoomba. This full-time role offers the opportunity to empower individuals, build meaningful relationships, and lead a dedicated team of support workers.

**About the Role**

As a Service Coordinator, you will:
**Client-Focused Support**: Build trusting relationships with clients and their families, understanding their unique needs and ensuring exceptional, person-centered care.

**Leadership and Mentorship**: Lead, coach, and mentor a team of support workers, fostering a positive work culture and professional growth.

**Service Coordination**: Oversee personalized care plans and collaborate with internal teams to ensure seamless service delivery and facilitate the onboarding process for new participants ensuring a smooth transition into the company’s services

**Client Relationship Management**:build and maintain strong, trusting relationships with participants, their families, key decision makes and other stakeholders such as support coordinators, OPG, behavioural practitioners etc

**Compliance and Documentation**: Maintain accurate client records, ensuring compliance with NDIS and regulatory requirements.

**Advocacy**: Advocate for clients to access necessary services and benefits, empowering them to make informed decisions about their care.

**What We’re Looking For**

To be considered for this role we are looking for someone who has the following:

- A minimum of **2+ years’ experience** in a similar role within the disability, aged care, or community services sector.
- Strong leadership and communication skills to inspire and guide a team.
- Demonstrated experience of training and mentoring teams of support workers
- Knowledge of the NDIS and experience delivering person-centered services
- Clinical Care Experience (preferred)
- A current NDIS Worker Screening Check, First Aid and CPR Certificate, and a valid Queensland driver’s license.
- Experience in service coordination for individuals with behaviours of concern on PBSP will be highly regarded
- You will also need a current NDIS Worker Screening Card, Drivers License and full work rights in Australia

**Location**

This role is located in Toowoomba and the successful applicant will be required to make daily visits to our SIL participants and visit other participants. This role can therefore be considered a hybrid work from home role as you will also be required to work from our SIL Villas on allocated days of the week.

**How to Apply**

Pay: $38.00 - $40.00 per hour

Expected hours: No more than 38 per week
