This full-time contracted position requires someone skilled in developing and overseeing customized support plans that cater to the diverse needs of our clients. As a Support Coordinator, you will play a pivotal role in providing comprehensive, client-focused solutions that empower individuals to lead more fulfilling lives.

**Key Responsibilities**:
As a Support Coordinator, you will play a pivotal role in empowering our clients by:

- **Assessing their needs** and coordinating appropriate services that enhance their ability to live more independently.
- **Navigating complex systems** to break down the barriers that our clients face due to structural and systemic issues.
- **Innovating solutions** and thinking outside the box to address unique challenges our clients face daily.
- **Documenting and reporting** with precision - your ability to synthesize and communicate complex information will be crucial.
- **Managing tasks efficiently** in a fast-paced environment, ensuring that competing priorities are addressed, and deadlines are met without compromising quality.

**Required Qualifications & Skills**:
To excel in this role, you must have:

- A tertiary qualification in Social Work or Nursing.
- Eligibility for full membership with the Australian Association of Social Workers (AASW) or Australian Health Practitioner Regulation Agency APHRA.
- Proven experience in service coordination for individuals with complex psychosocial and disability needs.
- A valid driver’s license, Working With Children Check, Right to work in Australia, and NDIS Worker Screening Check.
- Exceptional documentation and report writing skills.
- A high level of independence, with the initiative to find innovative solutions.
- The ability and enthusiasm to work as part of a dynamic team, supporting colleagues and celebrating shared successes.
- Strong organizational skills, with a flair for managing tasks and meeting deadlines in a dynamic environment.

**Salary**: $70,000.00 - $75,000.00 per year

Expected hours: 20 - 38 per week
