Are you passionate about making a difference in the lives of individuals receiving NDIS and Aged Care support? Nightingale Home Care are seeking a dedicated and compassionate **Client Care Coordinator** to join their team. This full-time role is key to ensuring clients receive high-quality, tailored care while fostering positive relationships and satisfaction.
**Role Overview**:
As a **Client Care Coordinator**, you will serve as the primary point of contact for all client relations, overseeing client intakes, referrals, and the management of important client documentation such as service agreements and personal care plans. You will be responsible for conducting in-home visits and scheduling regular meetings with clients and their families to ensure ongoing communication and satisfaction.
This position involves close collaboration with our care teams to ensure a deep understanding of each team member’s skillset and services available to our clients. You will also play a critical role in maintaining accurate and up-to-date client records, contributing to the seamless delivery of services. Additionally, you will work closely with the Rostering Team to effectively accommodate client requests and ensure the best possible care experience.
**Key Responsibilities**:
- Serve as the main contact for all client relations, including intakes, referrals, and documentation management.
- Conduct in-home visits and schedule regular meetings with clients and families.
- Ensure that all client records, including service agreements and care plans, are accurate and current.
- Maintain ongoing communication with clients and their families to monitor satisfaction and address concerns.
- Collaborate with the Rostering Team to meet client needs and preferences.
- Report directly to your Department Manager to ensure service delivery aligns with organisational goals and standards.
**Key Requirements**:
- Experience in NDIS or aged care services is essential.
- Experience in Care Coordination is essential.
- Strong interpersonal and communication skills, with the ability to build trust with clients and families.
- Exceptional organisational skills and attention to detail, especially in managing client records and documentation.
- Ability to work independently and as part of a collaborative team.
- A compassionate and client-centred approach to care.
