**About the Role**
The Program Leader works alongside Program Workers, role modelling behaviours and practice that aligns with a wellness and reablement approach, to ensure that programs and new initiatives are person-centred. Other key responsibilities of the position include:
- Leading and supporting a team in planning and implementing activities that foster client wellbeing and reablement
- Managing referrals, conducting assessments and putting in place interim support strategies where needed
- Preparing programs, organising necessary equipment and resources to undertake activities
- Responding to feedback from clients and their families by working with the Team Leader to develop programs and new initiatives that are appealing to older people across a wide age spectrum
- Monitoring client health and wellbeing and reporting any changes in status to the primary carer and/or family member
- Accurately reporting on statistical client-related data in a client management system, with ability to monitor progress and achieve funding targets
- Liaising with other service delivery teams internally and externally to respond to service needs of our participants.
- Monitoring and maintaining compliance in accordance with The Aged Care Quality Standards, Food Safety Plans, Commonwealth Home Support Program and Quality System Operating Procedures
**Skills & Experience**
- Tertiary qualification in human services areas, such as in allied health, disability or related discipline (desirable)
- Aged Care Certificate or lesser formal qualifications with relevant experience and work skills
- Experience working with people from diverse backgrounds including those with memory loss, disability, cultural diversity and social isolation.
- Level 2 First Aid Certificate
- Excellent communication skills
- Advanced skills in time management
- Demonstrated experience in program development
- Established computer skills and willingness to learn specific software programs
- Current Victorian Driver’s Licence and willingness to drive clients in a 12-seater minibus
- Bi-lingual skills and ability to speak a community language is (highly desirable)
