**The Role**

The Program Coordinator is a pivotal role within Your LINC South, located in Noarlunga, serving the Southern Adelaide Local Health Network (SALHN). The position oversees three key support programs:

- ** ED Crisis Service**: Provides short-term support for individuals in psychosocial distress, helping resolve issues and connect to long-term services.
- ** Intensive Crisis Support (ICS)**: Offers high-intensity case management to reduce distress, support transitions, and focus on long-term wellbeing
- ** Non-acute Psychosocial Support (NAPS)**: Delivers moderate-intensity case management to promote independence, connect to community supports, and assist with NDIS transitions.

The Program Coordinator primarily manages the ED Crisis Service, with occasional involvement in other support packages. Reporting to the Your LINC South Manager, the Program Coordinator is responsible for ensuring high-quality, responsive service delivery and managing a client caseload. You will monitor risk and quality management systems, attend case conferences, mentor staff, and maintain strong relationships with clients and stakeholders.

The following key responsibilities will ensure you contribute to the program meeting its objectives.
- Monitor and coordinate incoming referrals to ensure suitable client-staff matching.
- Participate in case conferences and client reviews, supporting team members, clients, and networks.
- Maintain a client caseload, delivering individual or group support.
- Ensure accurate and up-to-date case notes and documentation.
- Mentor and inspire best practices within the team, promoting a positive and productive work environment.
- Build strong relationships with key stakeholders.
- Support site resource development and review to meet operational needs.

**What you need to bring**
- Certificate IV in Mental Health or equivalent qualification.
- Experience in mental health services, working with individuals with complex needs.
- Experience in recovery frameworks and trauma-informed care.
- Ability to make decisions under pressure and manage crises.
- Knowledge of community services and the NDIS eligibility and transition processes.
- Degree in Social Work, Psychology, Counselling, or a related field. (Desirable)
- Experience with Athena Penelope electronic data management system. (Desirable)

**Special Condition**

Willingness to take part in Centacare’s fleet car program or grey fleet your personal vehicle.
