We are presently looking for the right people to provide care, support and assistance to a number of clients across Brisbane.
**About your role**:
As a Community Care Worker (Home Care Worker), you’ll work with clients in their homes, following their care plans, to provide support and ensure their needs are met.
Daily duties include:
- Assisting clients with their care requirements in accordance with their care plans and preferences.
- Supporting clients with transportation and community access.
- Monitoring their condition and reporting on any changes in condition or behaviour.
- Ensuring confidentiality of clients.
- Providing in-home personal care and daily living assistance, manual handling and domestic assistance.
**About you**:
To be successful in this role, you will:
- Have a Certificate III in Individualised Support (or studying towards) or experience working alongside clients with cognitive and/or physical limitations.
- Possess a caring, compassionate and supportive nature with strong communication skills.
- Be able to work in a self-directed environment
- Have intermediate ability to use IT systems including MS Word.
- Have your own mobile phone.
- Have your own vehicle and a Queensland Provisional or above clear Driver’s Licence.
- Have the ability to gain the required government legislative documents.
