**Job Title: Support Coordinator - NDIS (Contractor Role)**
**Location**: Perth
**Employment Type**: Independent Contractor.

We are now seeking a **Support Coordinator** to join us on an **as-needed, contractor basis**ideal for someone looking to supplement existing work or pick up occasional referrals with flexibility and autonomy.

**Key Responsibilities**:

- Support participants to understand and implement their NDIS plans.
- Connect participants with services and monitor outcomes.
- Coordinate service bookings and provider relationships.
- Assist with documentation, progress reports, and review preparation.
- Build strong, respectful relationships with participants and their networks.

**Requirements**:

- Experience delivering Support Coordination under the NDIS.
- Strong understanding of the NDIS framework and provider landscape.
- ABN and contractor readiness.
- NDIS Worker Screening Check.
- National Police Clearance.
- Excellent communication and organisation skills.
- Relevant qualifications (e.g. Certificate IV or higher in Community Services, Mental Health, Disability - preferred).

Pay: $61,150.50 - $102,673.02 per year
