We are looking for experienced professionals to join our team to undertake non-clinical client assessments that simplify and improve access to aged care services and support clients to live independently.

**About the Role**
As an Aged Care Assessment Officer (Non-Clinical) you will assess clients eligible for Commonwealth Aged Care Services.
- Undertake low level/non-clinical assessments fir people that are 65 years and older (50 years or older for Aboriginal or Torres Strait Islander people) who require assistance with everyday tasks
- Review and developing support plans to assist clients in achieving their goals and living independently in their own homes and community
- Facilitate effective engagement and participation and encourage client choice and involvement in decision making when developing the detailed care plans to meet their needs.
- Utilise an Integrated Assessment Tool (IAT), to enable referrals for Commonwealth funded services for older people living at home
- Meet individual and team KPIs and targets to meet funding requirements
- Identify, monitor and manage risk and lead appropriate escalation.
**Skills & Experience**
Essential
- Demonstrated understanding or experience in aged care or age care assessment
- Understanding of My Aged Care Gateway and the Commonwealth funded services for older people living at home
- Experience working within client reporting management systems
- Current Victorian driver’s licence, Working with Children Card and fully vaccinated against COVID-19
