**About the Role**
The Home Support Unit supports 2,500 Darebin residents with a range of services delivered in the home and community. The key aspect of the Service Planning Team is to manage the rosters of 100+ Community Support Workers to ensure services are delivered efficiently and effectively. To be successful in this role, you will be customer focused and have experience working with aged and/or people living with disabilities. You will have a good understanding of Aged Care services and extensive experience in using a client management system, such as Carelink+.
- Provide Age Friendly and diversity aware customer focus
- Complete rostering and service allocation for large workforce
- Collaborate to achieve team goals in a fast-paced environment

**Skills & Experience**
- An understanding of the Commonwealth Home Support Programme
- Extensive experience in rostering services for in home aged care and disability services
- Experience using a client management system such as Carelink plus
- Proven problem-solving skills, and ability to find creative solutions
- Thrives in a high-pressure working environment
- Excellent written and verbal communication
- Current Working with Children Check and Police Check
